REVENUE MEMORANDUM CIRCULAR NO. 84-2019 issued on August 22, 2019 notifies the loss of several unused BIR Form No. 0423 (Apprehension Slip), to wit:

Serial Number APS200100018051 – APS200100018100 Quantity 1 pad
Serial Number APS200100018102 – APS200100018150 Quantity 49 sets

The abovementioned forms, which were reported as lost by former Administrative Section OIC-Chief Emerita A. Teleron of Revenue District Office No. 114, have been cancelled and all official transactions involving the use thereof are considered as invalid.
