The abovementioned forms were reported as lost by Revenue Collection Officer Abygail Nicole O. Rodrigo of Revenue District Office No. 41. All official transactions involving the use of the lost copies of the issued ROR should be verified. With respect to the lost unused/unissued sets, the same have been cancelled and all official transactions involving the use thereof are considered as invalid.
