REVENUE MEMORANDUM CIRCULAR NO. 8-2004 issued on February 18, 2004 notifies the loss of one (1) triplicate copy of an unused BIR Form No. 1954 – Certificate Authorizing Registration with Serial No. CAR200300222435.

  Said form, which was reported lost by Mr. Juan T. Trono of Revenue Region No. 6, Manila, has been cancelled and all official transactions involving the use thereof is considered invalid.
