REVENUE MEMORANDUM CIRCULAR NO. 8-2002 issued on February 26, 2002 notifies the loss of three (3) sets of used BIR Form No. 0421-Tax Verification Notice with Serial Numbers TVN200000156132-TVN200000156134.
     Said forms, which were reported lost by Revenue District Office No. 1 – Laoag City, Ilocos Norte, have been cancelled and all official transactions involving the use thereof are therefore considered as invalid.
