REVENUE MEMORANDUM CIRCULAR NO. 73-2017 issued on September 8, 2017 notifies the loss of one (1) pad of unused BIR Form No. 0535-Taxpayer Information Slip bearing Serial Nos.TIS201400298801 to TIS201400298850.

     Said forms, which were reported as lost by Ms. Leonarda S. Vicente of Revenue District Office No. 36, have been cancelled and all official transactions involving the use thereof are considered invalid.
