Serial Number ROR201301822087 – ROR201301822100 Quantity 14 sets
Serial Number ROR201301818989 – ROR201301819000 Quantity 12 sets
Serial Number ROR201301822628 – ROR201301822650 Quantity 23 sets

The abovementioned forms, which were reported as lost during the Marawi siege in Marawi City by Administrative Assistant Jamel A. Boriongan of Revenue
District Office No. 102, have been cancelled and all official transactions involving the use thereof are considered as invalid.
