REVENUE MEMORANDUM CIRCULAR NO. 70-2019 issued on July 9, 2019 notifies the loss of one (1) unused set of BIR Form No. 0423 – Apprehension Slip with Serial Number APS200100000572.

Said form, which was reported as lost by Ofelia N. Felipe of Revenue District Office No. 27, has been cancelled and all official transactions involving the use thereofare considered as invalid.
