REVENUE MEMORANDUM CIRCULAR NO. 69-2026 issued on June 22, 2026 notifies all internal revenue officials, employees and others concerned about the loss of three (3) sets of unused/unissued BIR Form No. 0535 – Taxpayer Information Sheet bearing Serial Numbers TIS202100084948 to TIS202100084950.

Said forms, which were reported as lost by Revenue Officer Regine S. Boquiren of Revenue District Office No. 3, have been cancelled and all official transactions involving the use thereof are considered as invalid.
