REVENUE MEMORANDUM CIRCULAR NO. 69-2014 issued on September 8, 2014 notifies the loss of one (1) set of used but unissued BIR Form No. 2313 – Certificate Authorizing Registration (CAR) with Serial No. CAR201000335047.

        Said form, which was reported as lost by Ms. Lilia G. Herno of Revenue District Office No. 54B, has been cancelled and all official transactions involving the use thereof are considered invalid.
