REVENUE MEMORANDUM CIRCULAR NO. 67-2007 issued on October 30, 2007 notifies the loss of five (5) sets of unused BIR Form No. 19.65 – Letter of Authority (LOA) with Serial Numbers 158116 to 158120.

         Said forms, which were reported lost by Ms. Mylene M. Dungala of Revenue District No. 9, Revenue Region No. 2, Baguio City, have been cancelled and all official transactions involving the use thereof are considered invalid.
