REVENUE MEMORANDUM CIRCULAR NO. 65-2026 issued on June 22, 2026 notifies all internal revenue officials, employees and others concerned about the loss of two (2) sets of unused/unissued BIR Form No. 0423 – Apprehension Slip bearing Serial Numbers APS200300042484 to APS200300042485.

Said forms, which were reported as lost by Revenue Officer Rodolf Schervier S. Valerio of Revenue District Office No. 6, have been cancelled and all official transactions involving the use thereof are considered as invalid.
