REVENUE MEMORANDUM CIRCULAR NO. 62-2017 issued on August 7, 2017 notifies the loss of one (1) unused set of BIR Form No. 0423-Apprehension Slip bearing Serial No. APS200100007590.

     Said form, which was reported as lost by Mr. Luis B. Bonilla of the National Investigation Division, has been cancelled and all official transactions involving the use thereof are considered invalid.
