REVENUE MEMORANDUM CIRCULAR NO. 62-2009 issued on October 27, 2009 notifies the loss of five (5) sets of unused BIR Form No. 1954 – Certificate Authorizing Registration (CAR) with Serial Nos. CAR200800155396 to CAR200800155400.
     Said forms were reported as lost by Ms. Cynthia S. Calleja of Revenue District Office No. 68, Revenue Region No. 10 – Legazpi City. Consequently, said forms have been cancelled and all official transactions involving the use thereof are considered invalid.
