REVENUE MEMORANDUM CIRCULAR NO. 61-2018 issued on July 5, 2018 notifies the loss of one (1) set of unused BIR Form No. 0535-Taxpayer Information Sheet bearing Serial No. TIS201400257907.

        Said form, which was reported as lost by Ms. Hidjarah B. Macarambon of Revenue District Office No. 98, has been cancelled and all official transactions involving the use thereof are considered invalid.
