REVENUE MEMORANDUM CIRCULAR NO. 61-2004 issued on October 1, 2004 notifies the loss of one (1) set of unused BIR Form No. 1954 – Certificate Authorizing Registration with Serial No. CAR200400093102.
           Said form, which was reported lost by Ms. Lucita G. Peralta of RDO No. 23, RR No. 4, San Fernando, Pampanga, has been cancelled and all official transactions involving the use thereof is considered invalid.
