REVENUE MEMORANDUM CIRCULAR NO. 60-2005 issued on October 25, 2005 notifies the loss of one (1) piece of Tax Remittance Advice with Serial No. 558706, which was reported lost by Ms. Maria Bernadette Y. Galura, Accountant of Technical Cooperation Council of the Philippines. All official transactions involving the use of said form are considered invalid.
