REVENUE MEMORANDUM CIRCULAR NO. 58-2004  issued on September 16, 2004 notifies the loss of one (1) duplicate copy of unused BIR Form No. 1954 – Certificate Authorizing Registration with Serial No. CAR200400131805.
           Said form, which was reported lost by Ms. Nanette M. Canillada of BIR-Finance Division, RR No. 9, San Pablo City, has been cancelled and all official transactions involving the use thereof is considered invalid.
