REVENUE MEMORANDUM CIRCULAR NO. 57-2018 issued on July 3, 2018 notifies the loss of ten (10) sets of unused BIR Form No. 0423-Apprehension Slip bearing the following Serial Nos.:

                                                          APS200100000628 – 1 set

                                          APS200100000638-APS200100000639 – 2 sets

                                           APS200100000641-APS200100000647 – 7 sets

Said forms, which were reported as lost by Mr. Alfredo O. Pagdilao, Jr. of Revenue District Office No. 27, have been cancelled and all official transactions involving the use thereof are considered invalid.
