REVENUE MEMORANDUM CIRCULAR NO. 56-2012 issued on September 19, 2012 prescribes the use of BIR Form No. 2309 – Notice of Payment Schedule (NPS) relative to the implementation of Revenue Memorandum Order No. 21-2012.                                                                                                       The printing and distribution of the said form shall be the responsibility of Collection Service. The form shall be accomplished in 5 copies to be distributed as follows:
                    Original                       –                     Value-Added Tax (VAT) Tax Credit Certificates
                                                                              (TCC) Holder-Applicant
                    Duplicate                    –                     Revenue Accounting Division
                    Triplicate                    –                     NPS Issuing Office
                    Quadruplicate            –                     Attached to the VAT TCC docket
                    Quintuplicate             –                     Commission of Audit
