REVENUE MEMORANDUM CIRCULAR NO. 55-2012 issued on September 12, 2012 notifies the loss of several accountable forms with pertinent information, to wit:

Said forms were reported lost by Mr. Anwar T. Abbas of Revenue District Office No. 92. Consequently, all official transactions involving the use of the lost triplicate to quintuplicate copies of the issued Revenue Official Receipt should be verified. With respect to the unissued sets, the same have been cancelled and all official transactions involving the use thereof are considered as invalid.
