REVENUE MEMORANDUM CIRCULAR NO. 54-2002 issued on December 11, 2002 amends RMC No. 25-2002 which notifies the loss of one (1) set of unused Tax Verification Notice (BIR Form No. 0421).
  The serial number of said lost form should be TVN200200024600 instead of TVN200100024600.
      Said form, which was reported lost by Ms. Mary Rose B. Meneses of RR 6- Manila, has been cancelled and all official transactions involving the use thereof is considered as invalid.
