REVENUE MEMORANDUM CIRCULAR NO. 53-2002 issued on November 28, 2002 notifies the loss of one (1) set of unused BIR Form No. 1954 (Certificate Authorizing Registration) with Serial Number CAR200100078662.
     Said form, which was reported lost by Ms. Yolanda Canlas-Santiago of RDO 39, has been cancelled and all official transactions involving the use thereof are considered as invalid.
