REVENUE MEMORANDUM CIRCULAR NO. 52-2006 issued on August 25, 2006 notifies the loss of a previously cancelled BIR Form No. 1954 – Certificate Authorizing Registration with Serial No. CAR200400049960.

     Said form was reported lost by Ms. Ma. Victoria N. Basco of Revenue Region No. 8, Makati City. All official transactions involving the use thereof are considered invalid.
