REVENUE MEMORANDUM CIRCULAR NO. 50-2025 issued on May 13, 2025 notifies the loss of two (2) sets of unused/unissued BIR Form No. 0423 – Apprehension Slip with Serial Numbers APS201600006881 to APS201600006882.

Said forms, which were reported as lost by Group Supervisor Ellen Joy D. Cailing of Revenue District Office No. 103, have been cancelled and all official transactions involving the use thereof are considered as invalid.
