REVENUE MEMORANDUM CIRCULAR NO. 50-2024 issued on April 2, 2024, notifies the loss of five (5) sets of unused/unissued BIR Form No. 0535 – Taxpayer Information Sheet (TIS) with Serial Numbers TIS202000095602 – TIS202000095606.

Said forms, which were reported as lost by Revenue Officer Sushmita L. Malubag of Revenue District Office No. 53B, have been cancelled and all official transactions involving the use thereof are considered invalid.
