REVENUE MEMORANDUM CIRCULAR NO. 50-2017 issued on July 18, 2017 notifies the loss of fourteen (14) sets of unused BIR Form No. 0535-Taxpayer Information Sheet bearing Serial Nos. TIS201200080737 to TIS201200080750.

     Said forms, which were reported as lost by Mr. Jayson G. Ellado of Revenue District Office No. 89, have been cancelled and all official transactions involving the use thereof are considered invalid.
