REVENUE MEMORANDUM CIRCULAR NO. 5-2005 issued on February 9, 2005 notifies the cancellation of four (4) sets of BIR Form No. 1954 – Certificate Authorizing Registration with Serial Nos. CAR200400243149, CAR200400247038, CAR200400247047 and CAR200400206395.
        Said forms were cancelled since the first three had missing quadruplicate copies while the last one had a blank but numbered triplicate copy as per report by Ms. Nanette M. Canillada of Revenue Region No. 09, San Pablo City. All official transactions involving the use of said forms are considered as INVALID.
