REVENUE MEMORANDUM CIRCULAR NO. 46-2009 issued on August 20, 2009 notifies the loss of three sets of unused BIR Form No. 1954 – Certificate Authorizing Registration (CAR) with Serial Nos. CAR2000900018521 to CAR200900018523.
      Said copies, which were reported missing by Ms. Lodiminda P. Rom of Revenue Region No. 9, San Pablo City, have been cancelled and all official transactions involving the use thereof are considered invalid.
