REVENUE MEMORANDUM CIRCULAR NO. 41-2022 issued on April 8, 2022 notifies the loss of one (1) set of unused BIR Form No. 0535 – Taxpayer Information Sheet (TIS) with Serial Number TIS201800498827.
                Said form, which was reported as lost by Revenue Officer Joyzhel Glaizc B. Saladaga and Group Supervisor Lucille Mae R. Etorma of Revenue District Office No. 92, has been cancelled and all official transactions involving the use thereof are considered as invalid.
