REVENUE MEMORANDUM CIRCULAR NO. 39-2002 issued on September 10, 2002 notifies the loss of one (1) pad of unused BIR Form No.19.14–Apprehension Receipt with Serial Number 073951-074000.
     Said forms, which were reported lost by Mr. Gerardo Pabello, Jr. of RDO 89, have been cancelled and all official transactions involving the use thereof are considered as invalid.
