REVENUE MEMORANDUM CIRCULAR NO. 35-2018 issued on May 18, 2018 notifies the loss of triplicate copy from one (1) set of unused BIR Form No. 0423-Apprehension Slip bearing Serial No. APS200400004924.

       Said form, which was reported as lost by Ms. Nelma Kristine G. San Gabriel of the Regional Investigation Division of Revenue Region No. 14, has been cancelled and all official transactions involving the use thereof are considered invalid.
