REVENUE MEMORANDUM CIRCULAR NO. 35-2015 issued on June 25, 2015 notifies the loss of twenty (20) sets of unused BIR Form No. 0535 – Taxpayer Information Sheet with Serial Nos. TIS201400086011 to TIS201400086030.
     Said forms, which were reported lost by Ms. Blessie Lyn B. Rodriguez of Revenue District Office No. 67, have been cancelled and all official transactions involving the use thereof are considered invalid.
