Said forms were reported lost by Mr. Jimmy V. Orejola of Revenue District Office No. 88. Consequently, all official transactions involving the use of the lost duplicate to triplicate copies of the issued Accountable Forms should be verified. With respect to the unissued sets, the same have been cancelled and all official transactions involving the use thereof are considered invalid.
