REVENUE MEMORANDUM CIRCULAR NO. 34-2005 issued on August 8, 2005 notifies the loss of three (3) original copies of unissued BIR Form No. 17.58 – Authorityto Cancel Assessment with Serial Nos. ACA97-000015620, ACA97-000015639 and ACA97-000015643.
           Said forms, which were reported lost by Ms. Anita G. Jacinto of RR 5, Valenzuela City, have been cancelled and all official transactions involving the use thereof are considered invalid.
