The abovementioned forms were reported as lost by Mr. Philip Demetrio Murillo Viduya former Collection Section OIC-Chief of Revenue District Office No. 39. Consequently, all official transactions involving the use of the lost copies of the issued ROR should be verified. With respect to the lost unissued copies, the same have been cancelled and all official transactions involving the use thereof are considered as invalid.
