REVENUE MEMORANDUM CIRCULAR NO. 31-2002 issued on August 23, 2002 notifies the loss of one (1) set of unused BIR Form No. 0421–Tax Verification Notice with Serial Number TVN200100025240.
     Said form, which was reported lost by Ms. Corazon Namocatcat of RDO 33, has been cancelled and all official transactions involving the use thereof is considered as invalid.
