REVENUE MEMORANDUM CIRCULAR NO. 30-2012 issued on July 5, 2012 notifies the loss of several accountable forms, to wit:

BIR Form No. 2524 (Revenue Official Receipt) ROR201003911562- ROR201003911600 39 duplicate to quintuplicate copies (issued)
Accountable form No. 51 (Official Receipt) ACF201000018901- ACF201000018950 50 duplicate and triplicate copies (Issued)

         The loss of said accountable forms was reported by Mr. Lloyd P. Amboayan of Revenue District Office No. 101. Consequently, all official transactions involving the use of the lost accountable forms should be verified.
