REVENUE MEMORANDUM CIRCULAR NO. 3-2004 issued on January 14, 2004 notifies the loss of one (1) Tax Credit Certificate issued to Asia Traders Insurance Corporation with Serial No. 000959.

     Said form, which was reported lost by Ms. Salvacion Ana R. Cruz, Accounting
Manager of above named taxpayer, has been cancelled and all official transactions
involving the use thereof are considered invalid.
