REVENUE MEMORANDUM CIRCULAR NO. 27-2012 issued on June 18, 2012 notifies the loss of one (1) unused set of BIR Form No. 0425 – Notice of Disqualification under NO AUDIT PROGRAM with Serial Number NOD200600004291.                                                                                                           Said form, which was reported missing by Ms. Lydia N. Brion of the Assessment Division of Revenue Region No. 9, San Pablo City, has been cancelled and all official transactions involving the use thereof are considered invalid.
