REVENUE MEMORANDUM CIRCULAR NO. 25-2018 issued on April 19, 2018 notifies the loss of five (5) sets of unused BIR Form No. 0535-Taxpayer Information Sheet bearing Serial Nos. TIS201100008865, TIS201100008866, TIS201100008867, TIS201100008868 and TIS201100210321.

        Said forms, which were reported as lost by Ms. Irene M. Calalang of Revenue District Office No. 27, have been cancelled and all official transactions involving the use thereof are considered invalid.
