REVENUE MEMORANDUM CIRCULAR NO. 25-2006 issued on April 19, 2006 notifies the loss of two (2) used and two (2) unused sets of BIR Form No. 1954 – Certificate Authorizing Registration with Serial Nos. CAR200300156260, CAR200400180650, CAR200500005695 and CAR200400317850.

     Said forms, which were reported lost by Ms. Virgilia A. Aguirre, Ms. Anabel S. Evangelista and Ms. Vivenciana G. Faustino of Revenue Region No. 5, Valenzuela City, have been cancelled and all official transactions involving the use thereof are considered invalid.
