REVENUE MEMORANDUM CIRCULAR NO. 25-2002 issued on July 30, 2002 notifies the loss of one (1) set of unused BIR Form No. 0421-Tax Verification Notice with Serial Number TVN200100024600.
     Said form, which was reported lost by Ms. Mary Rose Meneses of RR 6-Manila, has been cancelled and all official transactions involving the use thereof is considered as invalid.
