REVENUE MEMORANDUM CIRCULAR NO. 24-2013 issued on March 13, 2013 notifies the loss of fifty (50) unused sets of Accountable Form No. 51 – Official Receipt with Serial Nos. ACF201000776651 to ACF201000776700.

     Said forms, which were reported as lost by Ms. Fely I. Bonete of Revenue District Office No. 53B, have been cancelled and all official transactions involving the use thereof are considered invalid.
