REVENUE MEMORANDUM CIRCULAR NO. 22-2013 issued on March 4, 2013 gives notice on the missing duplicate copy from one (1) unused set of BIR Form No. 0421 – Tax Verification Notice (TVN) with Serial No. TVN200300158282.

     Said form was reported missing by Ms. Maria Belen S. Pajarillo of Revenue District Office No. 82. Consequently, the entire TVN set bearing the said Serial Number has been cancelled and all official transactions involving the use thereof are considered invalid.
