The abovementioned forms were reported missing by Ms. Myra M. Auman, Chief, Taxpayers Service Section/Support Head, One Time Transactions Unit of Revenue District Office No. 113 under Revenue Region No. 19, Davao City. Consequently, said forms have been cancelled and all official transactions involving the use thereof are considered invalid.
