REVENUE MEMORANDUM CIRCULAR NO. 19-2014 issued on March 19, 2014 notifies the loss of one (1) set of unused BIR Form No. 2313 – Certificate Authorizing Registration with Serial Number CAR201300200600.

          Said form, which was reported lost by Ms. Melba R. Ventura of Revenue District No. 44, has been cancelled and all official transactions involving the use thereof are considered invalid.
