REVENUE MEMORANDUM CIRCULAR NO. 19-2004 issued on March 22, 2004 notifies the loss of one (1) set of unused BIR Form No. 0421 – Tax Verification Notice with Serial No. TVN200100100129.

    Said form, which was reported lost by Ms. Cecile U. Deduro of Revenue District Office No. 53, has been cancelled and all official transactions involving the use thereof are considered invalid.
