REVENUE MEMORANDUM CIRCULAR NO. 18-2019 issued on January 25, 2019 notifies the loss of one (1) set of unused BIR Form No. 0402 – Authority to Cancel Assessment with Serial Number ACA200600011054.

Said form, which was reported as lost by previously designated Revenue District Officer Alexander P. Laroza of Revenue District Office No. 71, has been cancelled and all official transactions involving the use thereof are considered as invalid.
