REVENUE MEMORANDUM CIRCULAR NO. 17-2009 issued on March 31, 2009 notifies the loss of one set of used but unissued BIR Form No. 1965 – Letter of Authority with Serial No. LOA200800014131.
     Said form, which was reported missing by Mr. Jayser I. Avila of RDO No. 65, RR No. 10, Legazpi City, has been cancelled and all official transactions involving the use thereof are considered invalid.
