REVENUE MEMORANDUM CIRCULAR NO. 13-2014 issued on February 24, 2014 notifies the loss of six (6) sets of unused BIR Form No. 0402 – Authority to Cancel Assessment (ATCA) with Serial Nos. ACA200700021015 to ACA200700021020.

        Said forms, which were reported lost by Ms. Cynthia M. Catolico of Accounts Receivable Monitoring Division, have been cancelled and all official transactions involving the use thereof are considered invalid.
